GoldenCare ACO is a pioneering Accountable Care Organization dedicated to serving Medicare beneficiaries in long-term care facilities. Since its inception in 2024, GoldenCare ACO has been committed to meeting the unique needs of elderly individuals, as well as those managing Alzheimer's disease, dementia, depression, amputations, developmental or physical disabilities, renal failure, and multiple chronic conditions and complications.

We support long-term care physicians and facilities in adopting value-based care by conducting thorough analyses and offering actionable data and insights. Additionally, unlike other managed Medicare programs like Medicare Advantage Institutional Special Needs Plans (I-SNPs), providers within SilverCare ACO face no downside risk and are not required to make an initial capital contribution for participation.
